				How to Become a Member


				. Fill out the NCDS Membership Application. You can download and print the application by clicking here (if you are unable to open, click here), or you can request an application by contacting NCDS by phone 800-662-8754/ 919-677-1396, fax 919-677-1397 or email rducharme@ncdental.org 2. Fill out both pages of the application and return to NCDS. 3. Do not send payment with your application. When the application process is complete, a statement will be mailed to you. Once you receive the statement, you can mail a check payable to NCDS or pay or by phone using a Visa or MasterCard. 4. When you join NCDS, you are also joining the ADA and your district dental society. You do not have the option of joining only one organization.
